Social Prescribing Program

Improve your health through connection and activity with the SOS Social Prescribing Program. This program helps connect seniors to community-based services, programs, and supports that promote well-being, including social engagement, housing, transportation, and financial resources. Eligible participants can also engage with others and learn about local resources at the SOS Seniors’ Gathering Place. Seniors with low to modest incomes who can participate independently, or who are experiencing loneliness or social isolation, may qualify. Building connections and staying active can improve both physical and mental health. Learn more and see if you qualify at sosd69.com/social-prescribing.
